Town and Country Manor is located in Boerne, Texas, at the entrance of the scenic Texas Hill Country. Our center is a licensed skilled nursing facility specializing in short-term medical and rehabilitation care as well as long-term care. We are dedicated to serving the special needs of our community and have responded over the years by developing new and specialized programs. Town and Country Manor is easily accessible and located near major acute care hospitals in the San Antonio area.
Short-term Services -
If you are seeking short-term medical and/or rehabilitative services, we will focus our efforts working closely with you and your family on achieving the most successful functional outcomes so that you can return home as quickly as possible. A dedicated team of healthcare professionals will develop an individualized care plan that is designed to address your specific needs. Supporting the nursing team is a group of licensed therapists who provide physical, occupational and speech therapies for patients recovering from major surgery, stroke, neurological and orthopedic conditions and other illnesses, injuries and disabilities. We have dedicated a special unit of 12 rooms designed exclusively for patients who have as their primary goal to return home as quickly and as safely as possible. We are guided by the principle that quality healthcare is best achieved when we work together.
Long-term Care Services -
Our long-term care units are designed for individuals who need 24-hour nursing care and may be chronically ill, frail or experiencing a very slow recovery from an illness or injury. These units offer private and semi-private room accommodations including room furnishings, color TV, and beautiful common areas. In addition, we offer respite or vacation care that allows families an opportunity to take a brief break from the demands of caring for a loved one round-the-clock.
We accept Medicare, Medicaid, managed care insurance, Veterans Administration, and private pay.
